Also called a bilateral mastectomy, double incision mastectomy involves opening the skin on the chest using two horizontal incisions—one at the top and another at the bottom—of the pectoral muscle and removing breast fats, tissues, and excess skin. The nipples and areolas may also be decreased in size and repositioned for a natural-looking flat chest.

Keyhole top surgery, using the periareolar approach, makes a semi-circular incision along the base of the areola and the breast tissue is removed using liposuction. This top surgery technique leaves the nipple stalk intact, allowing for a higher chance of maintaining nerve sensation. Despite the desirable benefits, keyhole top surgery isn’t for everyone. Only about 5% of the general population are good candidates for this procedure.
